History:

* NY 21's original alignment began at the PA line near Troupsburg and followed NY 36's current alignment to Hornell. NY 21 and NY 36 swapped alignments south of Hornell sometime around the mid 1950s.
* Prior to 1937, NY 21 followed present day Steuben CR 121 and NY 371 through the Cohocton area.
* In 1980, NY 21's northern terminus was moved from Lake Rd. in Pultneyville to NY 104 in Williamson. The decommissioned section is now maintained by Wayne County.
* In 1981, NY 21 in Canandaigua was moved off Bristol St. onto the new Canandaigua Bypass, joining US 20/NY 5. Along Main St., NY 21 was extended south to the intersection with the new bypass.
