The existing alignment of NY 34 pretty much follows the original alignment. Both terminii have changed over the years. The 1954 Sunoco map of New York shows the southern terminus at Chemung St., in Waverly, which carried NY 17 at the time. The northern terminus is shown at NY 370 in Cato. With the construction of the Southern Tier Expressway in the early 1970's, NY 34 was extended south to the PA line via Cayuta St. (The NY 17/NY 34 interchange sits just a few feet from the state line). At the north end, NY 34 was extended north to NY 3 in Hannibal, then truncated back to NY 104 when the highway was bypassed around Hannibal in the 1960's.

In Ithaca, NY 34, along with NY 13 and NY 96, followed Meadow St. through downtown Ithaca. At State St. (NY 79), NY 13 and NY 34 turned east and followed NY 79's current westbound alignment to Stewart Ave. NY 34 then followed Stewart Ave. north to East Shore Dr. where it would rejoin its present alignment. In 1969, NY 34 was moved to its present alignment when the NY 13 expressway north of the city was completed. In 1998, Meadow St. was switched from two-way to one way traffic as part of the Octopus reconstruction project. NY 13 and NY 34 southbound traffic was moved to nearby Fulton St.
