NY 286A was originally assigned as a roughly quarter mile spur of the original NY 286 in Washington County into Poultney, Vermont along what is now Washington CR 18A. The original alignment would be renumbered as the eastern end of NY 273 in the mid 1940s when the parent route was renumbered as NY 22A.

The more recent incarnation of NY 286A was assigned to Browncroft Blvd. in Rochester, Brighton, and Penfield in 1949. The designation was shifted to Blossom Rd. in the early 1960s and remained there until it was removed in the early 1970s.
