Present day NY 286 was assigned in 1949, originating in downtown Rochester at East Ave. (NY 96) and following E. Main St., University Ave., and Blossom Rd. before joining the current alignment in Penfield. In the early 1960s, NY 286's alignment shifted north from Blossom Rd. to its current alignment on Browncroft Blvd. Its western terminus was also truncated to the Rochester city line at an interchange with the newly built NY 47 (now NY 590) expressway.
