NY 391 was previously assigned to Whiskey Hill Rd. (Seneca CR 107) between NY 96 and current NY 318 (then NY 291) in Junius, Seneca County.

The current alignment was previously assigned as US 219 before its current expressway alignment opened to traffic in the mid 1970s.

NY 391 previously formed a useless duplex with US 62 in Hamburg. It was eliminated with NY 391's truncation to its current northern terminus in 2004.
