NY 3A was originally assigned to present day NY 31 between Middleport and Medina in Orleans and Niagara Counties. In 1932, the designation shifted to portions of the NY 93 and NY 425 alignments west of Lockport in Niagara County and remained there until 1935 when the parent route's western terminus was truncated to Cayuga County.

Present day NY 3A was assigned in 1950. There is speculation that NY 3A could be a candidate for decommissioning in the near future due to plans to expand nearby Fort Drum.
